COI Requirements for bands

I am looking for examples of Certificate of Insurance (COI) Requirement guidelines or matrix that your school uses to determine if/how much insurance is required for bands. I am hoping to gather enough examples and information to help our Risk Management  develop our own guidelines. If you would be willing to share your documents or knowledge it would be much appreciated!

Here are the insurance requirements for Valparaiso University. This is demanded by legal counsel and our Insurance Company.

 

$1,000,000.00- individual

$2,000,000.00- aggregate

$300,000.00- for damages

$5,000.00- minimum for medical

$1,000,000.00- minimum vehicle insurance

 

We require everyone to name us as an additional insured. I hope this is helpful. If you need anything else please let me know.

Thanks for your response. Is this required of all bands, regardless of type of music and venue?

We require it of all major concert acts no matter what venue they are in on campus. I try to get insurance from everyone, but sometimes you have local small bands that have no insurance and they can't afford to get the insurance.
